The COVID-19 pandemic has affected us all and we are doing all that we can to protect British Columbians. There is a coordinated effort underway to learn about COVID-19 and to look at how we can further improve care for people who have experienced it. Research plays a big part in this effort. You can play a role in research by agreeing to be contacted to participate in future studies about COVID-19.

This registry is for people who have tested positive for COVID-19 and are interested in participating in COVID-19 research.

If you choose to participate:

• Approved BC researchers may contact you about COVID-19 research by phone, text message, email, or mail.

• The information you provide will be added the BC Centre for Disease Control (BCCDC) COVID-19 Consent Registry, maintained by the BCCDC and securely stored at the University of British Columbia.

• Your personal information (e.g. date of birth) will be used to link to results of your COVID-19 test and other information held by BCCDC (e.g. mailing address, date of COVID-19 diagnosis, PHN).

• Access to the information you provide will be carefully controlled by the BCCDC to ensure that you are matched with appropriate research projects and to avoid you from being over contacted.

• The information you provide will also be linked to REACH BC, an electronic platform endorsed by the BC Ministry of Health that helps connect persons diagnosed with COVID-19 to researchers across the province.

Both the BCCDC COVID-19 Consent Registry and the REACH BC platform are fully compliant with the BC Freedom of Information and Protection of Privacy Act ("FIPPA"). Your information is kept secure, and is stored and backed up in Canada.

You must be over the age of 18 to provide your consent to participate in the COVID-19 Consent Registry. Your participation is entirely voluntary and will not affect any health care services or other government services you currently receive or in the future. If you change your mind at any time, you can remove your information from either the BCCDC COVID-19 Consent Registry or the REACH BC platform by contacting or 1-855-707-2469.

By submitting the required information to participate, you agree that consent has been given.

Your personal information is collected under the authority of section 26 (c) of FIPPA. The information you provide will only be used for the purposes outlined above. Questions regarding the collection of your personal information or requests for records may be directed to the Information Access Privacy office that supports BCCDC at or 604-707-5833.
